Pertinent Test Question Submission Form – For Confirmed Appointments Only

Please submit your proposed pertinent test questions as soon as possible for review. Due to established laws and blood pressure cuff time limits, there is a strict limit of up to (4) pertinent test questions that may be established by the client for each polygraph examination (all other questions on the polygraph test are generic in nature and are established by the examiner for baseline purposes). The questions you submit will be reviewed by an examiner usually within 24-48 hours from receipt of your proposed questions. Any required reformatting, corrections and or suggestions will be sent to you via email prior to your actual appointment (time permitting). Otherwise, the questions and any reformatting will be discussed and reviewed during your actual appointment. No consultations are provided via telephone. Following are the required guidelines to assist you in establishing and wording the questions. Please read the guidelines carefully:

1. Questions need to be about a PRIOR ACTION/STATEMENT (something that a video camera could have recorded)
2. Questions need to be worded so they are all answerable “YES” or “NO” only
3. Questions need to focus on the same type of subject matter
4. Questions need to be worded so they are short, concise, specific and to the point
5. Questions CAN NOT be about Emotions, Intentions, Thoughts or Future Actions
